The Italian connection is meaningless. A nurses istant profession is not sufficient to migrate and you'd not be able to get the qualification prior to turning 45 (you need a 4 years Bachelor of Nursing degree) so you are out for being the primary applicant for the migration visa application. If your wife is under 45 then being a doctor might be OK if you apply right now. Leaving it past June might be problematic as doctors are shortly to be removed from the migration list and there is a new list coming out on July 1. She will need to p IELT and once here will under go a medical test followed by another intern like year which will mean she is supervised prior to being given an full medical license.
